Managing your condition in New York

Our care managers are here to help you live well.

NY Anthem’s Condition Care is all about you

Our Condition Care (CNDC) programs allow you to ask questions about your condition, get benefit information, and connect with local resources.

What programs are available for members?

We offer healthcare and support services for members with many health conditions.

Cancer Care Navigator

Managing all the pieces of your care after being diagnosed with cancer can feel overwhelming. Our Cancer Care Navigator program can help. It provides you with a team of nurses and other healthcare providers to help support your care. We’ll work with your doctors to simplify everything and make sure you receive the cancer care you need at no extra cost.

Cancer Care Navigator will also connect you to helpful resources. Some of those include a dietitian, help with managing medications, customized care plans, goal setting, and support after hours.

Get back time to focus on your well-being with extra support. Learn more about how the program can help you: Call the Cancer Care Navigator team toll free at 833-649-0669 (TTY 711) Monday through Friday, 8 a.m. to 9 p.m., and Saturday, 9 a.m. to 7:30 p.m. Eastern time.

Taking part in the program is optional, but encouraged. Choosing not to take part in the program does not impact your benefits.

How it works

Our CNDC care managers are registered nurses. When you join one of our CNDC programs, a care manager will:

Help you create health goals and make a plan to reach them.

Coach you and support you through one-on-one phone calls.

  Track your progress.

Give you information about local support and caregivers.

Answer questions about your condition and/or treatment plan (ways to help with health issues).

Send you materials to learn about your condition and overall health and wellness.

Coordinate your care with your healthcare providers, like helping you with:

  • Making appointments.
  • Getting to healthcare provider visits.
  • Referring you to specialists in your health plan, if needed.
  • Getting any medical equipment you may need.

Who can join?

New York Anthem Blue Cross and Blue Shield HP members with any of the health conditions listed above can participate in a CNDC program. Our CNDC care manager will talk with you about your health history, health goals, and work with you to find solutions so you can live well.

Care managers also offer tools for weight management and tobacco cessation (how to stop using tobacco such as quitting smoking).
